Downloadable Forms        

Below are downloadable forms used for client intake. Please print a copy of the Intake and Treatment Consent forms to read, fill out, initial and sign. Please bring these to your first session. If we are working with insurance, then you will need to contact your insurance company to inform them that you will be working with me and to find out your copay and deductible amounts (if any). Copays are due at the start of each session. If there is a deductible on your account, then I will charge you the full insurance rate at each session and provide you with a receipt so you can be reimbursed from your insurance directly.

Regarding the Intake form: Short answers are sufficient. I use your answers to guide my interview. I will be gathering more information in the session.
